Jefferson Center seeking RFPs

The 1889 Jefferson Center for Population Health, established through a partnership between the 1889 Foundation and the Jefferson College Of Population Health, has released a request for proposals seeking partners to serve as care coordination agencies under the Pathways Community HUB model in Cambria and Somerset counties.
